Do I need to submit proof of my qualifying life event?

  • For several types of qualifying life events, proof is required. If proof is required, we need to receive your proof within 10 calendar days from the date you submitted your application or Account Change Form or before your special enrollment period ends, whichever comes first.
  • In most cases, proof of documents must be dated within 60 days of your qualifying life event.
  • You should receive a confirmation letter within 14 days. If it says any proof is missing or incorrect, you’ll need to submit the required proof within 30 calendar days of the date of the letter.
  • If we don’t get your proof in time, we’ll have to cancel your application or account change request. You may apply again if you’re still within your special enrollment period.
  • If you apply through Connect for Health Colorado, check their website for their proof rules.

For Colorado - Loss of minimum essential health coverage

If you lost coverage from your employer, send in one of these types of proof:

If you lost coverage from your carrier or Health First Colorado, Child Health Plan Plus (CHP+), Medicare, or other government programs, send in one of these types of proof:
