Do I need to submit proof of my qualifying life event?

  • For several types of qualifying life events, proof is required. If proof is required, we need to receive your proof within 10 calendar days from the date you submitted your application or Account Change Form or before your special enrollment period ends, whichever comes first.
  • In most cases, proof of documents must be dated within 60 days of your qualifying life event.
  • If we don't get your proof in time, we'll have to cancel your application. You may apply again if you're still within your special enrollment period.
  • If you apply through Covered California, check their website for their proof rules.

For California - Loss of minimum essential health coverage

If you lost coverage from your employer, send in one of these types of proof:

  • Former health/medical record number listed in your application if your most recent health coverage was a Kaiser Permanente plan. We will review your prior coverage in our membership system.
  • Letter or other document from your employer stating that the employer dropped or will drop coverage or benefits for you, your spouse, or dependent family member and the date this coverage ended or will end
  • Letter or document from your employer stating that the employer stopped or will stop contributing to the cost of coverage and the date when this contribution ended or will end
  • Letter or document that indicates your coverage is ending due to age
  • Pay stubs of both current and previous hours if a reduction in work hours caused you to lose coverage

If you lost COBRA coverage, send in one of these types of proof:

  • Letter showing your employer’s offer of COBRA coverage or stating when your COBRA coverage ended or will end. We must receive your application within 60 days of the date when your COBRA coverage will end as stated on your proof.
  • Proof from your employer or COBRA administrator showing subsidies had been provided and the date they will end.

If you lost coverage from your carrier or Medi-Cal, Children’s Health Insurance Program (CHIP), Medicare, or other government programs, send in one of these types of proof:

  • Letter from your carrier showing a coverage end date
  • Letter or notice from Medi-Cal, or Children’s Health Insurance Program (CHIP) stating when Medi-Cal, or Children’s Health Insurance Program (CHIP) coverage ended or will end
  • Letter or notice from a government program, like TRICARE, Peace Corps, AmeriCorps, or Medicare, stating when that coverage ended or will end

If other loss of coverage including individual coverage, send in one of these types of proof:

  • If your most recent health coverage was through Kaiser Permanente include your former medical record number in your application. We will review your prior coverage in our membership system.
    • If your most recent Kaiser Permanente enrollment was through Covered California, also include a letter or other document stating your health coverage has ended or will end.
  • Letter from school stating when student health coverage ended or will end.
  • Letter or other document from Social Security office stating that the person who covers you on their health plan is entitled to Medicare.
  • Letter or other document from an employer stating that the person who covers you on their health plan is starting new employer coverage.
  • Dated military discharge papers or Certificate of Release, including the date that coverage ended or will end, if you’re losing coverage because you’re no longer on active military duty
  • Dated and signed written verification from an agent/broker/producer or dated letter from the carrier, if you are or were enrolled in a non-calendar year plan that's ending, including the date the plan ended
